Die Sprechapraxie wird im Allgemeinen definiert als zentralmotorische Störung des Sprechens (vgl. Böhme 2003, 348). Im Gegensatz zur Dysarthrie ist die Sprechapraxie eine artikulatorische Störung, hervorgerufen durch eine Beeinträchtigung der sprechmotorischen Programmierung. „Betroffen ist die Transformation des Outputs von der (abstrakten) phonologischen Ebene zur (konkreten) motorischen Ebene (Aichert/ Staiger 2005, 1)“. Laut Darley et al. ist die Fähigkeit, willentlich die Positionen der Sprechmuskulatur zur Produktion von Phonemen zu programmieren, gestört. Das Gleiche gilt für die Programmierung von Bewegungssequenzen für die Produktion von Wörtern (vgl. Darley, Aronson & Brown 1975 zit. nach Aichert/ Staiger 2005, 3).
Poeck definiert die Apraxie allgemein als „ ...eine Gruppe von Störungen, die das Ausführen von Bewegungen oder Bewegungsfolgen sowie das zweckmäßige Hantieren von Objekten betreffen, und die nicht durch Lähmung, Ataxie, Dyskinesie, Sensibilitätsstörung oder Beeinträchtigung im Sprachverständnis oder im Erkennen von Objekten bedingt sind (Poeck 2002, zit. nach Aichert/ Staiger 2005, 3)“.
Die Sprechapraxie gilt heutzutage als eigenständiges Syndrom und wird klinisch abgegrenzt von aphasischen Störungen, Dysarthrien und bukkofazialen Apraxien (vgl. Aichert/ Staiger 2005, 3), sowie ganz allgemein von den Gliedmaßenapraxien, die in sich noch unterteilt werden in Gangapraxien, Ankleideapraxien, konstruktive Apraxien (Probleme beim Abzeichnen und Konstruieren) und unimodale Apraxien (vgl. Pritzel et al. 2003, 262).
Traditionell wird unterschieden zwischen der ideomotorischen und der ideatorischen Apraxie.
Dies geht zurück auf die Anfänge der Apraxie- Forschung durch H. Liepmann zu Beginn des 20. Jahrhunderts.
Der ideatorischen Apraxie liegt, so die ursprüngliche Bedeutung, das Fehlen eines Plan der Bewegung zugrunde. Sie wird also bezogen auf den Entwurf einer Handlung.
Der Begriff der ideomotorischen Apraxie beschreibt ganz allgemein eine Störung von Handlungen ohne Objekte. Dies bezieht sich zum Beispiel auf die Produktion kommunikativer Gesten oder das Imitieren von Bewegungen (vgl. Aichert/ Staiger 2005, 3).
Hier ist die Umsetzung eines Entwurfs oder Plans in ein motorisches Programm gestört (vgl. Pritzel et al. 2003, 262).
Die Sprechapraxie wird der Gruppe der ideomotorischen Apraxien zugeordnet.
